Four Adams County Jail Officers Charged After Inmate Found Unresponsive in Soiled Holding Cell

Four Adams County Jail corrections officers have been charged after an inmate was found unresponsive in a booking-area holding cell on April 20. All four face misdemeanor dereliction of duty charges; one officer, Kiara Williams, also faces a falsifying-records charge. Investigators say the inmate was discovered amid dried feces, urine and vomit and was hospitalized with "serious medical issues" attributed to being left unattended. An independent investigation concluded documented checks were falsified.

Four corrections officers at the Adams County Jail have been charged after an inmate was reportedly left unattended in a booking-area holding cell for several days and later hospitalized, according to an affidavit and statements from investigators.

The officers charged are:

Terry Akers, 40; Danny Kiskadden, 36; Chad Stanforth, 41; and Kiara Williams, 21. Court records show each faces one count of misdemeanor dereliction of duty; Williams is additionally charged with falsifying records.

Timeline and allegations:

According to the affidavit and the Clermont County Sheriff’s Office, jail staff moved the inmate from his assigned housing unit after he allegedly became combative on April 14. He was taken to the Adams County Regional Medical Center after a change in behavior, evaluated, and returned to the jail. Between April 17 and April 20 the inmate remained in a booking-area holding cell.

On April 20, staff discovered the inmate unresponsive in the cell. The affidavit states the area around him contained dried feces, urine and vomit. He was transported to the hospital and diagnosed with what investigators described as "serious medical issues that persisted from being left unattended and unchecked."

Clermont County Sheriff Chris Stratton said investigators were asked to conduct an independent probe after the hospital diagnosis.

Investigators found that, despite log entries indicating physical checks were performed, all four officers did not check on the inmate during their shifts over the four-day period. Williams is accused of entering false check records into the jail’s system, which accounts for the additional charge against her.

Legal action: Formal charges were filed July 28 in Adams County Municipal Court. The Warren County Prosecutor’s Office is handling the prosecution. Media partner Fox19 and the Cincinnati Enquirer reported on the case.
